NUNN-WOOTEN SCOUTING FELLOWSHIP

Created by the NFL in January 2015, introduces players to a career in professional scouting. The program gives participants a glimpse into player personnel by exposing them to college and pro scouting.

Participants shadow members of an NFL team's scouting staff and learn:

• How to evaluate players
• How to improve their ability to spot talent

LINE OF DUTY DISABILITY (LOD)

Credited Seasons Requirement: Not an Active Player & no specific credited seasons requirement

Application deadline: Former Players with 0-4 Credited Seasons must apply within 48 months after the end of their last contract with an NFL Club as a Player. Former Players with 5+ Credited seasons have equal to their number of Credited Seasons to apply (e.g., 6 Credited Seasons = 6 years of eligibility)

Description: A Player may be entitled to an LOD benefit if the Disability Initial Claims Committee or the Retirement Board determines that, as a player, you have (1) incurred a substantial disablement arising out of NFL football activities, and (2) timely applied for this benefit. NEUROCOGNITIVE DISABILITY (NC BENEFIT)

Credited Seasons Requirement: Vested Players with 4+ Credited Seasons pre-1993, Vested Players with 3+ Credited Seasons post-1993, and Non-Vested Players who apply within 84 months after the end of their last contract with an NFL Club. 

Application deadline: Non-Vested Players have 84 months after the end of their last contract with an NFL Club as a Player. Vested Former Players can apply until they begin collecting pension benefits.

Description: A Player may be entitled to an NC benefit if at least one Plan neutral physician selected finds that the Player has a mild or moderate neurocognitive impairment. There are additional qualifications for the NC Benefit. TOTAL AND PERMANENT DISABILITY (T&P)

Credited Seasons Requirement: Active Player or Vested Inactive Player (additional requirements exist, please contact the Plan Office to determine eligibility).

Description: A Player may be entitled to T&P if the Disability Initial Claims Committee or the Retirement Board determines that (1) you are substantially prevented from or substantially unable to engage in any occupation or employment for remuneration or profit, and (2) your condition is permanent. 88 PLAN

Credited Seasons Requirement: Players with 4+ Credited Seasons pre-1993, and Players with 3+ Credited Seasons post-1993.

Description: A Player may be entitled to receive 88 Plan benefits if the 88 Committee or the 88 Board determines that (1) you are a vested player, and (2) your condition meets the 88 Plan's definition of Dementia, ALS, or Parkinson's Disease. The Plan reimburses or pays for medical and custodial expenses related to the aforementioned diagnoses. There are additional qualification criteria for the 88 Plan. THE NFL PLAYER TUITION ASSISTANCE PLAN

Reimburses eligible players for tuition, fees, and books – up to the maximum allowable benefit. More information on the Tuition Assistance Plan can be found HERE.

Active Player Eligibility Requirements

• Minimum of 1 Credited Season before the Semester begins.

• Must be on a game roster in a Qualifying Status (Active, Inactive or Injured Reserve) for the regular season or postseason.

• Eligible for up to $20,000 per Plan Year (September 1 – August 31).

Practice Squad Benefit Eligibility Requirements

• 3 games on Practice Squad in the season.

• $5,000 tuition available for that season.

The GUPAT Fund was created to provide short-term financial assistance to former players who may be faced with a financial or medical crisis. The GU PAT can also serve as a resource for former players that are interested in returning to school to complete their undergraduate degree or obtain a trade certification. The GU PAT funds are intended to provide immediate assistance when players are faced with unusual financial, medical, or educational challenges that may call for temporary assistance. CRITICAL CARE BENEFIT PROGRAM

DEDICATED HOSPITAL NETWORK PROGRAM (DHN)

Eligibility: Vested Former Players who are under age 65 and not currently receiving comprehensive medical benefits through the NFL Player Insurance Plan's 5-year Continuing Veteran coverage. There is no restriction based on their home address or proximity to a certain health system.

Description: A dedicated network of high-quality medical care providers in every NFL city offering Primary Medical Care and other services. The network is comprised of health systems selected by each NFL Club. Eligible Vested Former Players may seek treatment at any participating health system within the dedicated network. Services under this Program are provided free of charge up to the Annual Maximums (where applicable) to eligible Vested Former Players and cover 100% of Preventive Care, up to $25,000 per Plan Year for Primary Medical Care (including certain orthopedic services) and up to $25,000 per Plan Year for Mental Health Services. Annual Maximums renew September 1 each year until the last day of the month of an eligible Vested Player's 65^th Birthday.

DISCOUNT PRESCRIPTION DRUG CARD

Eligibility: Any Vested Former Player not covered under the NFL Player Insurance Plan, and their Dependents. 

Description: The benefit is a Discount Drug Card, which provides immediate discounts for prescription drugs at any pharmacy in the United States participating in the Card Provider's network. You cannot use the Discount Drug Card for over-the-counter or nonprescription medication. HEALTH REIMBURSEMENT ACCOUNT (HRA)

Credited Seasons Requirement: 3 + Credited Seasons available to Vested Former Players not currently under continuing Veteran coverage under the NFL Player Insurance Plan.

JOINT REPLACEMENT PROGRAM

Eligibility: Vested Player.

Description: If you are covered by insurance, the Basic Benefit is the lesser of (i) $5,250 or (ii) your co-insurance for health care items or services directly related to your surgery, such as your co-pays, provided the expense was incurred within one year of your surgery. If you are not covered by insurance and do not have the means to pay for this surgery, you may qualify for assistance from the NFL Player Care Foundation. The Plan only provides Benefits to first time replacements of a knee, hip, or shoulder. Contact the Benefits Administrator to verify eligibility.

NFL PLAYER INSURANCE PLAN - CONTINUING VETERAN "CV" COVERAGE

Credited Seasons Requirement: Vested Player (within 5 years of last date under contract).

Service Benefit: Vested Former Players and their eligible dependents receive same coverage as active players for 5 years post career at no cost, then option to purchase extended insurance coverage. NFL MEDICARE SUPPLEMENT BENEFIT

Eligibility: Any Vested Former Player who is at least age 65 and who has both Medicare Parts A and B coverage. 

Description: At age 65 or older, players are eligible to receive a monthly subsidy of up to $160 when in enrolling in a Medicare supplement policy available to Former Players through the Aon Retiree Health Exchange. Beginning on January 1, 2026, the monthly subsidy will increase to $200.

SUPPLEMENTAL MENTAL HEALTH BENEFIT

PCF / TULANE SCHOOL OF MEDICINE HEALTHY MIND & BODY SCREENING

Services: The NFL Player Care Foundation (PCF) has partnered with Tulane University School of Medicine to conduct its Healthy Body and Mind Screening program. This FREE national screening program is open to all former NFL players and their spouse/significant other. The screening includes a series of private and confidential cardiovascular screenings, mental health resources and education, sleep apnea education and an at-home sleep study, as well as comprehensive bloodwork. This bloodwork includes a metabolic panel, lipid panel, and various other biomarkers of thyroid function and overall cardiovascular health. NFL Player Care Foundation screenings are offered as part of PCF's research programs, which help to advance public awareness and scientific understanding of health issues that affect former NFL players.

*The former NFL player must be registered and attend the screening in order for the spouse/significant other to be eligible.
