Former Players with 5+ Credited seasons have equal to their number of Credited Seasons to apply (e.g., 6 Credited Seasons = 6 years of eligibility)
Description: A Player may be entitled to an LOD benefit if the Disability Initial Claims Committee or the Retirement Board determines that, as a player, you have (1) incurred a substantial disablement arising out of NFL football activities, and (2) timely applied for this benefit. NEUROCOGNITIVE DISABILITY (NC BENEFIT)
Credited Seasons Requirement:Â Vested Players with 4+ Credited Seasons pre-1993, Vested Players with 3+ Credited Seasons post-1993, and Non-Vested Players who apply within 84 months after the end of their last contract with an NFL Club.Â
Application deadline:Â Non-Vested Players have 84 months after the end of their last contract with an NFL Club as a Player. Vested Former Players can apply until they begin collecting pension benefits.
Description: A Player may be entitled to an NC benefit if at least one Plan neutral physician selected finds that the Player has a mild or moderate neurocognitive impairment. There are additional qualifications for the NC Benefit. Description: A Player may be entitled to receive 88 Plan benefits if the 88 Committee or the 88 Board determines that (1) you are a vested player, and (2) your condition meets the 88 Plan's definition of Dementia, ALS, or Parkinson's Disease. EDUCATION
THE NFL PLAYER TUITION ASSISTANCE PLAN

Description:Â A dedicated network of high-quality medical care providers in every NFL city offering Primary Medical Care and other services. The network is comprised of health systems selected by each NFL Club. Eligible Vested Former Players may seek treatment at any participating health system within the dedicated network. Services under this Program are provided free of charge up to the Annual Maximums (where applicable) to eligible Vested Former Players and cover 100% of Preventive Care, up to $25,000 per Plan Year for Primary Medical Care (including certain orthopedic services) and up to $25,000 per Plan Year for Mental Health Services. Annual Maximums renew September 1 each year until the last day of the month of an eligible Vested Player's 65th Birthday.

Description: If you are covered by insurance, the Basic Benefit is the lesser of (i) $5,250 or (ii) your co-insurance for health care items or services directly related to your surgery, such as your co-pays, provided the expense was incurred within one year of your surgery. If you are not covered by insurance and do not have the means to pay for this surgery, you may qualify for assistance from the NFL Player Care Foundation. Description: At age 65 or older, players are eligible to receive a monthly subsidy of up to $160 when in enrolling in a Medicare supplement policy available to Former Players through the Aon Retiree Health Exchange. Beginning on January 1, 2026, the monthly subsidy will increase to $200.

PCF / TULANE SCHOOL OF MEDICINE HEALTHY MIND & BODY SCREENING
Services: The NFL Player Care Foundation (PCF) has partnered with Tulane University School of Medicine to conduct its Healthy Body and Mind Screening program. This FREE national screening program is open to all former NFL players and their spouse/significant other. The screening includes a series of private and confidential cardiovascular screenings, mental health resources and education, sleep apnea education and an at-home sleep study, as well as comprehensive bloodwork. This bloodwork includes a metabolic panel, lipid panel, and various other biomarkers of thyroid function and overall cardiovascular health. NFL Player Care Foundation screenings are offered as part of PCF's research programs, which help to advance public awareness and scientific understanding of health issues that affect former NFL players.
*The former NFL player must be registered and attend the screening in order for the spouse/significant other to be eligible.
